Medicare Part B covers annual wellness visits 100% every 12 months.
This telephone assessment, performed by a licensed U.S. Nurse Practitioner, follows an evidence-based framework to assess your health risk and determine which preventive services are recommended for you this year. This allows you to stay up to date on the latest recommendations.
Your annual wellness visit includes:
- Private consultation with a licensed practitioner by phone, tablet, or computer.
- Screenings for memory loss, depression, fall risk, functional status, and more.
- Customized Medicare prevention plan outlining 5-10 yrs. of covered services for your physician.
- Report on additional Medicare covered benefits based on your health.
- Discuss concerns and questions with a Nurse Practitioner and more!
Not sure if you are eligible?
You and your loved ones are eligible for this free medical visit if you have:
- Minimum one year of Medicare Part B coverage,
- Completed “Welcome to Medicare” visit,
- No previous annual wellness visit this year.
